Home Visiting Service

NICS provides a Paramedic Practitioner lead home visiting service, with 5 – 6 response vehicles, dedicated to serving patients in North West Surrey who are housebound and not able to get to their GP Surgery.

What Is Led Home Visiting?

A team of dedicated Home Visiting Paramedic Practitioners are on the road during core GP hours, providing home or care home-based consultations for the housebound patients of North West Surrey.

Appointments are booked though the usual route of the patient's GP practice, who liaise with NICS who then coordinate the Paramedic visit. We follow NHS "Right Care, Right Place, Right Time" best practice.

How To Request A Home Visit

The service is for patients who are medically housebound, unable to attend their GP surgery.

Find A Hub
1

Contact Your GP Practice

Call your registered GP practice to request a home visit.

2

Clinical Assessment

The practice will assess whether a home visit is clinically appropriate, usually via a phone call.

3

Paramedic Home Visit

If suitable, the GP will request the visit with the Paramedic Home Visit Service. The GP is in communication with the Paramedic during the visit via secure NHS communications.

Please consider whether a phone or video call with your GP can resolve your problem without a visit. This will help ensure that patients who require an urgent face-to-face assessment can be seen promptly.

What Is The Role Of A Paramedic?

Paramedic Practitioners are highly trained clinicians who can assess, diagnose, arrange prescribed medication, coordinate referral into other service and arrange admission. The Paramedics will conduct a detailed assessment and then work with your GP to agree a treatment plan. They can also undertake point of care tests such as Blood Glucose or 12 lead ECGs and can obtain urgent bloods for laboratory testing if required. Your GP remains at the centre of your care at all times, with the paramedic conducting the assessment, which is documented in real-time for your GP to review. If hospital admission is required, the paramedic will liaise directly with the hospital team and arrange the admission.
